Assumption of Risk For Minors
In order to maintain compliance with all NCAA rules and regulations and all liability practices within the University System, Georgia Southern University Athletic Training has put in place the following procedures for ALL intercollegiate athletic participants.Â
All eligible athletes that are trying out for a GSÂ intercollegiate team must show proof of medical physical examination clearance within the past six months of the tryout date as per NCAA rules and regulations, as well as sickle cell results, in order to participate in the tryout. You can go to the GS Student Health Center to obtain sickle cell results. Allow one week for results to be returned.
Georgia Southern University athletic tryout participants must show proof of medical insurance in order to participate in the tryout. A copy of the front and back of the insurance card is required prior to the tryout time.Â
Georgia Southern University athletic tryout/evaluation participants must sign an athletic tryout release and an acknowledgment form in order to participate in the tryout/evaluation. If the participant is under 18 years of age, the Minor Assumption of Risk form must be filled out in place of the standard form.Â
The certified athletic trainer in charge of the particular sport will keep the required documents given to them by the athletic tryout participant until the coach has made a decision as to whether to accept the participant or deny.Â
Any tryout participant who makes their respective team will then be given a second physical examination by a Georgia Southern University Team Physician. This physical will be required prior to any official participation with the respective team.Â
If it has been determined the athletic tryout participant will NOT be a member of the respective team, all paperwork will be electronically filed for up to one year after the date of the tryout.
