Required to participate in any Georgia Southern sports camp:
1. Copy of insurance card (front and back)
2. A physical examination form signed by physician within one year from start date of camp
3. Signed release, covenant not to sure and assumption of risk form (available HERE).
Georgia Southern University Athletics camps are open to any and all entrants (limited only by number, age, grade level and/or gender).
