GEORGIA SOUTHERN FOOTBALL SUMMER 2010 FOOTBALL CAMP
Required to participate in any Georgia Southern football camp:
1. Copy of insurance card (front and back),
2. A physical examination form signed by physician within one year from start date of camp
3. Signed release, covenant not to sure and assumption of risk form
