Please be sure to read through these explanations thoroughly and familiarize yourselves with the athletic training policies and procedures at Georgia Southern University. Your signature at the end each document indicates that you have read and understood the information contained herein. A signature is required by the parent/guardian and student-athlete under the age of 18 for all forms.
- Athletic Training Policies and Procedures / Parent Letter: The purpose of this form supplies student-athletes and/or parent/guardian with thorough explanations of the policies and procedures of the Athletic Training department. Each student-athlete and parent/guardian must sign this form in order to participate in GSU athletics. The following information can be obtained in this form:
- Requirements for intercollegiate participation
- GSU's Financial Responsibility for injuries
- Policies for filing injury claims
- Parent/Student-Athlete Insurance Information Form: This form supplies GSU with your health insurance information. Please be certain to fill out each line of this form. If the information does not apply to you, please put an "N/A" on the line. Please remember to copy the front and back of your insurance card and affix it to the provided area on this form. Each student-athlete must sign this form in order to participate in GSU athletics.
- Medical History and Medical History Follow-Up Form: This form supplies GSU with your current medical history. Please fill out this form completely. Each student-athlete must sign this form in order to participate in GSU athletics.
- Assumption of Risk/Release Form: This form must be signed in order to participate, and by signing this form you indicate that you are aware of and accept the potential physical hazards of being a student-athlete. Each student-athlete must sign this form in order to participate in GSU athletics.
- ADD/ADHD Medication Exemption Information: This form provides explanation of the proper documentation required by the NCAA if the student-athlete is being treated by medication for ADD/ADHD. A FAQ and guidelines are provided for further explanation. GSU Athletic Training department has a form that will be need to be filled out and signed by the treating physician. This form must be submitted prior to participation with the following requirements:
- Brief summary of the comprehensive clinical evaluation used to diagnose this student athlete with ADD/ADHD and any supporting documentation.
- Please attach note-worthy alternative non-banned medications that have been tried or considered and why they were not utilized.
- Please attach any ADHD Rating Scale (ex. Connors, ASRS, CAARS) scores and report summaries.
- Follow-Up Medical Exemption Information: If GSU has already received initial documentation, the NCAA rules and regulations regarding ADD/ADHD medication requires a yearly follow-up examination. This form will need to be filled out and signed by the treating physician.
- Please attach a copy of the written prescription of the ADD/ADHD medication for our records.
- Please attach note-worthy alternative non-banned medications that have been tried or considered and why they were not utilized.
- Female Athlete Questionnaire: This form assists us in identifying concerns specific to female student-athletes. Each female student-athlete must sign this form in order to participate in GSU athletics.
